Description

Biktarvy is a prescription HIV-1 medication in tablet form. Each Biktarvy pill combines three antiviral drugs (bictegravir, emtricitabine, and tenofovir alafenamide) into a single, once-daily tablet. This one-pill regimen works by preventing HIV from multiplying in the body, which lowers the amount of virus (viral load) in the blood. Biktarvy should be taken exactly as prescribed by a healthcare provider. It can be taken with or without food. Note that Biktarvy does not cure HIV/AIDS; it helps manage the infection and prevents it from weakening the immune system

Indications and Usage

When taken daily, Biktarvy helps keep the virus under control. It is important to use it only as prescribed; Biktarvy is not for preventing HIV and should not be used for HIV pre-exposure prophylaxis (PrEP)

How it Works

Biktarvy contains three active agents that block HIV at different stages: bictegravir (an integrase inhibitor) and emtricitabine plus tenofovir alafenamide (both reverse transcriptase inhibitors). Integrase is an HIV enzyme that inserts viral DNA into human DNA; bictegravir blocks this step. Reverse transcriptase is another HIV enzyme needed to copy the virus; emtricitabine and tenofovir insert into the viral DNA chain to stop replication. Together, these actions prevent HIV from multiplying, which lowers the virus levels in the body

Dosage & Administration

  • Typical dose: The usual adult dose is one Biktarvy tablet (50 mg/200 mg/25 mg) taken by mouth once daily. For children who weigh 14–25 kg (about 30–55 pounds), a lower-dose tablet (30 mg/120 mg/15 mg) is given once daily. Dosing for smaller children should be determined by a healthcare provider.

  • With or without food: Biktarvy may be taken with or without food. To promote consistent levels of the medicine, try to take it at the same time each day.

  • How to take: Swallow the tablet whole. (For the lower dose tablet in children, it may be split in half and each half taken immediately, finishing within 10 minutes.) Do not crush or chew the tablet.

  • Follow instructions: Take Biktarvy exactly as prescribed. Do not change your dose or stop taking it without first talking to your doctor. If you miss a dose, take it as soon as you remember unless it’s almost time for your next dose; then skip the missed dose and resume your regular schedule.

Side Effects

As with any medication, Biktarvy can cause side effects. Many people tolerate it well, but some may experience:

  • Common side effects: Diarrhea, nausea, and headache are the most frequently reported side effects (about 5–6% of patients). Other common effects (in ≥2% of patients) include feeling tired, abnormal dreams or insomnia, dizziness, and abdominal discomfort. These often improve over time as the body adjusts.

  • Serious side effects (rare): Although uncommon, Biktarvy can cause more serious reactions. These include allergic reactions (symptoms such as rash, hives, itching, or swelling, especially of face/lips/throat). Other serious concerns are kidney problems (e.g. changes in urination, swelling of feet/ankles), lactic acidosis (a dangerous build-up of lactic acid causing weakness, breathing trouble, or stomach pain), and severe liver problems (symptoms like abdominal pain, yellow eyes/skin, dark urine). If any of these occur, medical attention should be sought immediately.

Warning and Precaution

  • Do not stop abruptly: If you have hepatitis B infection, suddenly stopping Biktarvy can cause severe hepatitis flare-ups. Your doctor will monitor liver function if you discontinue Biktarvy.

  • Kidney and liver monitoring: Biktarvy is not recommended for people with severely reduced kidney function (creatinine clearance <30 mL/min). Kidney and liver tests are usually done before and during treatment, because tenofovir can affect these organs. Tell your doctor if you have a history of kidney disease, Fanconi syndrome, or chronic hepatitis.

  • Lactic acidosis: Components of Biktarvy can, in rare cases, cause lactic acidosis and liver enlargement with fat (hepatomegaly with steatosis). Symptoms include feeling very weak, muscle pain, trouble breathing, stomach pain, nausea, irregular heartbeats, or unusual tiredness. Seek medical help if these occur.

  • Allergic reactions: In rare cases, a severe allergic (hypersensitivity) reaction may occur. Seek emergency care if you experience rash, hives, facial or throat swelling, or difficulty breathing.

  • Drug interactions: Some medications can interfere with Biktarvy. Do not take Biktarvy with certain drugs like dofetilide or rifampin. Tell your doctor about all prescription, over-the-counter, and herbal medicines you take, especially seizure drugs, TB drugs, St. John’s wort, or other anti-HIV meds.

  • Other precautions: Inform your doctor if you are pregnant, planning pregnancy, or breastfeeding. Biktarvy may be used in pregnancy if needed (pregnant patients usually require close monitoring). It is not yet known if Biktarvy passes into breast milk – a doctor can advise on the risks and benefits.
